Unique Identifier Information

I am trying to upgrade my current version to a multiple line version, and I have read in the forums that you need to supply your UID when purchasing the software. I follows the link supplied in other posts about the UID, http://www.voiceguide.com/vghelp/html/UniqueIdentifier.htm, however, it gives me a 404 error.

Could you please let me know how to find the UID that you need, or relink the HTML page?
