Had A Hardware Failure - Need To Transfer Machines

We purchased a Voiceguide Enterprise 4 line license which has been working fine for years.

Over the weekend, the computer it resides on died and we are going to be transferring to another computer.

What do we need to send you to have the software reactivated on the new computer?
